Making An Appointment

If you have one of these insurance plans, we will need verify that nutrition services are covered. To do this, have your insurance card ready when you call us so you can provide us your information to verify benefits.  Do know that any quotes on benefits is NOT a guarantee of insurance payment and that all unpaid claims are your responsibility.

Plan to pay any co-pays or co-insurance fees at the time of your nutrition appointment.  We take all forms of payment including cash, check, Mastercard, Visa, American Express and Discover.

Other Insurance

We provide discounted services for the following insurance plans:  

  • Humana (not listed above)
  • Aetna*
  • American Specialty Health
  • Tricare

*Out-of-network benefits may provide reimbursement.  Call for details.

Prior to Making An Appointment

Before you make an appointment with us, contact our office to find out the details of the discount we offer for each plan. This way you are informed before you schedule your first visit. 

FOR ALL APPOINTMENTS BOOK ON/AFTER 10/10/17: Please make note that we must have a full business weekday day in advance cancellation / reschedule notice (note that we are closed Thanksgiving, Christmas Day, and New Years so those do not count as a business day.)  Failure to do so will incur a $30 cancellation fee.  We book our appointments out up to a month in advance, and last minute cancellations make it impossible to get another appointment put in your spot.  Cancellation fees cannot be charged through insurance so this fee would be your responsibility.
